Should Cockpit Video Recorders Be Mandated After the Air India Crash?

The tragic crash of Air India Flight AI171 on June 12, 2025, which claimed 260 lives, has sparked renewed debate about cockpit video recorders in commercial aviation.

The preliminary report from India’s Aircraft Accident Investigation Bureau revealed that the Boeing 787’s fuel control switches were moved to the “cutoff” position shortly after takeoff. This resulted in a loss of engine thrust and led to the fatal crash.

A cockpit voice recording captured one pilot questioning the other about the switch movement, with the response, “I didn’t.”

This ambiguity has fueled speculation and highlighted the limitations of current investigative tools. Should the aviation industry mandate cockpit video recorders to prevent such uncertainty?

Why Cockpit Video Recorders Make Sense


The AI171 crash exposes a critical gap in aviation safety data. Cockpit voice recorders (CVRs) captured the pilots’ exchange, and flight data recorders (FDRs) logged the fuel switch movements. However, neither system could confirm who acted or why.

Cockpit video recorders, especially with an over-the-shoulder view, could have shown which pilot touched the switches, resolving whether the action was intentional or accidental.

Aviation safety expert John Nance argues, “In the balance between privacy and safety, the scale tips toward safety.” Video evidence could have clarified the AI171 mystery instantly, speeding up the investigation.

The National Transportation Safety Board (NTSB) has pushed for video recorders since 2000, citing cases like EgyptAir Flight 990, where intentional pilot actions were suspected but unproven visually.

The AI171 case echoes this, with public speculation about pilot error or even suicide, despite both pilots being experienced and cleared of impairment.

A 2025 Australian crash report called cockpit video “invaluable,” and IATA Director General Willie Walsh, a former pilot, sees a “strong argument” for video to complement CVRs and FDRs.

Video could also deter errors or misconduct, encouraging pilots to follow protocols, much like body cameras influence police behavior.

With 241 passengers, crew, and 19 people on the ground killed, including former Gujarat Chief Minister Vijay Rupani, the AI171 crash demands accountability.

Video evidence could reduce public speculation and conspiracy theories, restoring trust in aviation safety. By visually confirming actions, video recorders could ensure faster, more accurate investigations, preventing prolonged uncertainty.

The Case Against Cockpit Video Recorders


Pilot unions, including the Air Line Pilots Association and the International Federation of Air Line Pilots’ Associations, have opposed cockpit video recorders, citing privacy concerns.

Pilots spend hours in the cockpit, and constant surveillance could capture personal moments unrelated to flight operations.

The unions fear footage could be misused, especially in high-profile cases like AI171, where leaks could fuel media sensationalism or unfairly blame pilots.

The Indian Commercial Pilots’ Association criticized the preliminary report’s “reckless” insinuations, highlighting the risk of misinterpretation.

Opponents also argue that existing CVRs and FDRs are sufficient. For AI171, the FDR confirmed the fuel switches moved to “cutoff” and back to “run,” while the CVR recorded the critical dialogue.

American Airlines pilot Dennis Tajer claims investigators “already have enough data” to determine crash causes in most cases, suggesting video offers only marginal benefits.

Implementing video recorders poses logistical challenges. Retrofitting aircraft with secure cameras and storage systems would be costly for airlines like Air India, already under scrutiny.

Ensuring footage remains confidential requires robust security, which may not yet exist. Moreover, video could be misinterpreted without context.

In AI171, a pilot’s hand on the switch wouldn’t necessarily clarify intent or reveal mechanical issues.

Striking a Balance for Safety


The AI171 Air India crash strengthens the argument for cockpit video recorders. The fuel switches, protected by a locking mechanism, require deliberate force, making accidental movement unlikely.

Video could have confirmed whether a pilot acted or if a mechanical fault occurred, addressing the ambiguity that has left investigators and the public searching for answers.

While privacy concerns are valid, strict protocols—such as encrypted storage and access limited to crash investigations—could mitigate risks. Consulting pilot unions to address concerns would ease implementation.

However, the ongoing AI171 investigation may yet resolve key questions. Voice identification and wreckage analysis could clarify the switch movement, and a possible mechanical issue, like the FAA-noted locking flaw, might shift focus to design fixes.

Still, the crash arguably strengthens the case for video recorders as a standard tool. Without them, similar incidents risk prolonged uncertainty, eroding public trust.

Mandating cockpit video recorders, with clear safeguards, could perhaps compliment existing recorders and allow faster answers.
